LONG TERM CARE
We understand that selecting the right facility and level of care can be an overwhelming task. The world of health care is ever changing, and the medical terminology can be confusing. If you don’t know what level of care would best meet your needs, we are available to meet with you personally to help make that decision. We can provide a health screen, meeting with you here at the facility or where you currently live, gathering medical information from you, your family, physician and other health providers to determine your current care needs. We offer two distinct levels of care within our long term care program:

Intermediate Care
Individuals who require nursing supervision and assistance with personal care find that the accommodations on our Intermediate Unit meet their needs perfectly. Those residing on the second floor join Residential Living residents in the Dining Room and enjoy a full schedule of activities designed just for them. Each spacious room has a private half bath and is fully furnished, but many residents choose to bring a few pieces of their own to make their new room feel like home.

Skilled Nursing Care
Our Skilled Nursing Unit is reserved for those who truly require 24-hour nursing care. Residents on this unit have their own private dining room, access to a solarium and outdoor terrace. The skilled unit is secure providing protection for those with Alzheimer's disease or other forms of dementia. Residents on our skilled unit participate in activities that are offered throughout the facility. Additional, special programs are designed for those who prefer one to one activities.

Our comprehensive services for Intermediate Care and Skilled Nursing include:

THE APPLICATION PROCESS AND THE WAITING LIST
If you are interested in applying for admission to Connecticut Baptist Homes, Inc. for Skilled Nursing or Intermediate Care:

Submit an application:
An application form may be obtained through the admissions office. 
Please call (203) 237-1206
and request an application from our Admissions Director.  
(Admission to Skilled Nursing or Intermediate Care requires a written application).

Upon receipt of the completed application, a clinical evaluation will be done and the admissions office will call to discuss availability. Please note that at times we are unable to admit at the time of application due to lack of available space. In that case, your name will be put on a wait list and you will be contacted as soon as an appropriate accommodation becomes available. At the time of admission, we will request orders for admission and a summary of the resident's current condition from the resident's physician.
